These form part of its chronic disease shared care pilot scheme.
As part of the Chronic Disease Common Care Pilot Scheme (CDCC Pilot Scheme), Hong Kong has introduced specialized care clinics and allied health services under the District Health Network.
These services follow a co-payment model, whereby eligible participants receive a partial subsidy from the government and pay a specified co-payment fee.
Specialized care clinic services provide assessment and treatment of diabetic foot disease, as well as personalized staged improvement plans for a healthier lifestyle and diet.
Allied health services include customized exercise programs and pain management solutions provided by physical therapists for complex cases such as joint pain in overweight individuals.
Targeted dietary guidance was also provided to participants who did not achieve expected improvement goals, while optometrists conducted fundus photography examinations for DM and HT patients.
These services are provided through strategic procurement at more than 40 service points across Hong Kong to support District Health Centers (DHCs) and DHC Express.
The rollout of these services will be phased, with non-participating DHC/DHCE members receiving specialized care clinics and allied health services in the next phase.
